Activa Pain Center ("we", "us") operates clinics at 22C, 4th Floor, Main Bukhari Commercial, DHA Phase 6, Karachi, and at Plot 367-C, Shahrah-e-Quaideen, PECHS Block 2, Karachi. Information we collect
Information you give us
- Your name, phone number and (optionally) email address
- Preferred clinic location, appointment date and time preference
- The condition you select and any symptom description you choose to write
- Any imaging reports or clinical documents you send us by email or WhatsApp
Information created during your care
- Clinical history, examination findings and diagnosis
- Imaging performed at our clinics, and imaging you bring from elsewhere
- Treatment records, functional measurements and review outcomes
- Billing and payment records
Information collected automatically
Our hosting provider records standard server logs, typically your IP address, browser type, the pages requested and a timestamp. These are used to keep the site running and secure.
If analytics, advertising pixels or a chat widget are added later, they will be listed here and a cookie consent mechanism added to the site before they are switched on.
As currently built, this website sets no advertising or analytics cookies. Fonts and JavaScript libraries are loaded from third-party content delivery networks, which necessarily receive your IP address in order to serve those files.

- To contact you about appointments, results and follow-up
- To prepare invoices and, where you ask us to, insurance documentation
- To meet our legal, regulatory and professional record-keeping obligations
We do not sell your information. We do not share it with third parties for their marketing purposes.

- Clinicians and staff within Activa involved in your care
- Other healthcare providers where you are referred, with your knowledge
- Your insurer, only where you ask us to submit a claim
- Service providers who host our systems, bound by confidentiality obligations
- Authorities, where disclosure is required by law
How long we keep it
Retention periods are set by our professional and legal record-keeping obligations. Write to us at the address below and we will tell you exactly how long your records are kept and why.
Security
Patient information is transmitted over encrypted connections and stored with access limited to staff who need it for your care. Physical records are held securely at the clinic. No system is perfectly secure; we will notify you and the relevant authority where required if a breach affecting your information occurs.
Your choices
You may ask us to provide a copy of the information we hold about you, correct anything inaccurate, or stop contacting you for non-clinical purposes. Write toinfo@activapaincenter.com. We may need to verify your identity before acting, and certain clinical records must be retained even where you ask us to delete them.
Children
Where a patient is under 18, appointment requests should be made by a parent or legal guardian, who exercises these rights on the patient's behalf.
Changes
We will update this policy when our practices change and revise the review date above. Material changes affecting how your information is used will be brought to your attention directly.
